I scanned about 90 tests yesterday and had only one anomoly. For one student, X's were given in 12 of the 20 questions. I scanned it again and received the same result. This student's sheet had no erasures of markings located outside of the ovals. 7 of the Xs were given where the student indicated the correct answer and 5 of the Xs given were for incorrect answers.

On another note, the scan worked much better for all my scans yesterday when I placed the 4 corner guides well within the scan sheet. Closer worked better.

When you looked at the results did the app "see" an answer on the questions that were marked wrong?  Sometimes the app doesn't pick up the marks and thinks the question is blank.   In these cases I grab a sharpie and fill in the bubbles that way  and try different lighting.
