I like the idea of a customisable ticket.  The free response option would need to be up to 6 marks for the exam I do at the moment, but it may vary for other subjects and exams.

Free response would obviously slow down the scanning process, but would allow for more differentiation of answers to test items.

On the part of the form that says 'DO NOT MARK', change this to a test number.  This would allow different tests to be undertaken in the same class for differentiation purposes.  Just using the circles already there you could have 1000 tests (including 0000).  If you added another column, it would allow for up to 10000 tests.  Allow in the scan for this not to be used if the same test is being taken by all students in the class.

You could provide a template to download in the format that the students names are expected.  This would make it more user friendly for teachers with lower computer skills.

I have rarely been excited about the possibilities of a new app, but Quickkey has enthused me to recommend this to all staff at my school.  I am going to try to sign up as many as possible when we return after the Easter break.
